Dutasteride Mesotherapy
Advanced Targeted Hair Loss Treatment by a Specialist Prescriber
Dutasteride mesotherapy is an advanced, targeted treatment for male pattern hair loss (androgenetic alopecia). It involves the micro-injection of dutasteride directly into the scalp in order to reduce local dihydrotestosterone (DHT) activity at the hair follicle level.
This treatment is designed to provide high follicular effect with reduced systemic exposure compared to oral DHT blockers.
All treatments are performed by a qualified Independent Prescribing Pharmacist with specialist experience in hair restoration medicine and PRP/mesotherapy techniques.
How Dutasteride Mesotherapy Works
Dutasteride is a potent 5-alpha reductase inhibitor, meaning it reduces conversion of testosterone to DHT — the primary hormone responsible for follicular miniaturisation in male pattern baldness.
Unlike oral treatment, mesotherapy delivers small quantities of medication:
- Directly into the dermal layer of the scalp
- Close to hair follicles
- With reduced systemic circulation compared to tablets
This targeted delivery aims to:
- Maximise scalp DHT suppression
- Minimise whole-body exposure
- Improve tolerability for some patients
Clinical Benefits Compared to Oral DHT Blockers
Compared to oral finasteride or dutasteride:
1. Targeted Action
- Mesotherapy acts locally at the scalp
- Oral treatments act systemically across the entire body
2. Potentially Reduced Systemic Side Effects
- Oral DHT blockers are associated with systemic hormonal effects in some patients
- Mesotherapy may reduce systemic absorption
3. Hair Growth Outcomes
Published studies on intradermal and mesotherapy-based dutasteride delivery show:
- Increased hair density and thickness
- Improved hair shaft calibre
- Stabilisation of progressive hair loss
A systematic review of dutasteride delivery methods found that both oral and intradermal dutasteride demonstrated clinically meaningful improvements in hair density and hair count in men with androgenetic alopecia.
A randomised controlled study using dutasteride-based scalp delivery systems has also demonstrated significant increases in hair count compared with placebo groups.
Evidence includes clinical trials and reviews published in dermatology journals evaluating intralesional and topical/microneedling-based dutasteride delivery systems in male androgenetic alopecia.
Key Published Evidence
- Systematic review of oral and intralesional dutasteride showed significant improvement in hair count and density in androgenetic alopecia
- Randomised controlled studies of scalp-delivered dutasteride (including microneedling-assisted delivery) show statistically significant hair regrowth vs placebo
- Clinical mesotherapy studies report improved hair thickness and density after repeated sessions, though protocols vary and larger standardised trials are still emerging
Overall, evidence suggests that dutasteride is one of the most effective pharmacological options for male pattern hair loss, and scalp-directed delivery is a developing technique aimed at improving tolerability and targeting.
Side Effects and Safety Profile
While mesotherapy is designed to reduce systemic exposure, some absorption can still occur.
Common (Local) Effects
- Redness and swelling at injection sites
- Mild scalp tenderness
- Bruising
- Temporary shedding (initial telogen effluvium phase)
Less Common
- Folliculitis
- Headache
- Mild irritation or itching
Systemic Effects (Rare but Possible)
- Reduced libido
- Erectile dysfunction
- Ejaculatory changes
- Breast tenderness or enlargement (gynecomastia)
- Mood changes (low mood or anxiety)
Important
Rare cases of persistent sexual side effects after stopping 5-alpha reductase inhibitors have been reported in medical literature, although the true frequency is unknown.
The risk of systemic effects with mesotherapy is thought to be lower than oral therapy, but cannot be completely excluded.
Why This Treatment is Only Offered to Men
This treatment is only suitable for adult men with androgenetic alopecia because:
Dutasteride is a potent hormonal medication affecting DHT metabolism
It is strictly contraindicated in pregnancy
Exposure during pregnancy may cause abnormal development of male fetal genitalia
The risk of teratogenic exposure makes it unsafe for women of childbearing potential
For safety and regulatory reasons, this treatment is therefore not offered to women.
Treatment Protocol
Our standard evidence-informed protocol is:
Initial phase: 1 session per month for 3 consecutive months
Maintenance phase: every 3–6 months, depending on response and hair stability
What Results Can Be Expected?
Most patients who respond may experience:
- Reduced hair shedding within 8–12 weeks
- Gradual improvement in density over 3–6 months
- Thicker, stronger terminal hairs
Slowing or stabilisation of further hair loss
However:
- Results are not guaranteed
- Advanced hair loss may have limited reversibility
- Ongoing maintenance is typically required
Important Information
Dutasteride mesotherapy is:
- An off-label medical treatment
- Still an evolving area of clinical practice
- Supported by emerging but not yet fully standardised protocols
